APN Renewals with Ohio Board of Nursing

Applications are mailed to the current address that is on file with the Board of Nursing. If you have changed your address since your last renewal, and have not notified the Board of the change, please do so immediately. When notifying the Board of your address change, please include your full name, nursing license number, or social security number.
